My aunt was in another facility prior to being at Country Oaks. Unfortunately, she had another stroke and needed to be placed in a nursing home. We chose Country Oaks because of its proximity to where my uncle lives. The facility is very nice inside and out. The staff and care are top-notch. The nurses from CNAs to supervisors are amazing, activities are always fun and the social services and case manager are great. One of the things my uncle said is that there were so many welcome phone calls when my aunt was first admitted that he could not remember the names of staff, but it showed how eager and caring they are. We also liked that they met with the family to discuss the care. Our family was so thankful to have found this place. Thank you all for taking such good care of my aunt.

She LOVED the cuisine! & made a joke recently about seeing if she could make a reservation, for Valentine’s dinner. We chose Country Oaks because of its proximity to where my uncle lives. The facility is very nice inside and out. The rooms can be updated, however, the staffs and care are top notch. The nurses from cnas to supervisors are amazing, activities are always fun and the social services and case manager are great. One of the things my uncle said is that there were so many welcome phone calls when my aunt was first admitted that he could not remember the names of staffs, but it showed how eager and caring they are. We also liked that they meet with the family to discuss the care. No one ever sat down with us to discuss the plans and we are very thankful for that. Our family was so thankful to have found this place. Thank you all for taking such good care of my aunt.

My grandmother was recovering at this hospital for a couple of months and whenever I’d visit there was this care taker she always kept talking to me about. How great and friendly she was and how she looked forward to getting to spend some time with her and the conversations they’d have, Elisabeth is the one I want to thank she brought the menu options for the day. The Dietary department; always providing great service and food, bringing a smile to the people I love, thank you.

I would like to say thank you to the Country Oaks staff for taking care of my husband during his recovery. I was a bit hesitant in putting him at a skilled nursing facility, but they helped him so much during his recovery. The LVNs and CNAs were great with attending to his needs, and rehab in helping him regain his strength. Activities always had some type of event going, and Social Services and case management were extremely helpful in setting up home health and getting him everything he needed to come home. I can’t thank you all enough for everything!

Great staff and very clean facility. My family has been really happy since our loved one has been a patient.

My father received top-notch professional care in this facility while recuperating from a serious illness. During the course of his 90-day stay, the staff went above and beyond to make sure he was well cared for. My dad took part in many activities that he really enjoyed. The activity director made sure that their residents are as active as they can be and take personal interest in their progress.

In addition to the recreational activities, my dad also participated in a physical therapy program that was beneficial to him.
I highly recommend this facility to anyone in a position needing long-term care.

First and foremost, I would like to thank all the staff at Country Oaks care Center. I would personally like to start off with the level of care for our loved ones that they take into consideration when it comes to the recovery. The activities director always organizes such amazing and awesome events for the residence. If you’re looking for a place for your loved one to have the great care when it comes to the recovery, I truly believe this is one of the few places I’d highly recommend , my husband has been at multiple facilities prior to Country Oaks care Center. The engagement that they have with the residence is undescribable they put so much time and organization when it comes to events for them I believe this place a huge role in the recovery. I am beyond grateful for all the staff here. Starting from the receptionist at the entrance to the end of the nurses in line. We’ve had a couple miscommunication incidents, but at the end of the day everything gets situated when it gets addressed… I love how the activities director organizes these type of activities not only for the residence, but for the families. I can truly say that I’m highly satisfied with the level of care of my husband has received. Not only do care and love for my husband, but they also have grown to love my family. Prior to Country Oaks I’ve always had incidents where we couldn’t get in touch with the higher-ups when it comes to the level of care for my husband here at Country Oaks it seems as if any situation arises there’s someone always there to address any matters. I would personally like to leave a review for Yesenia, the head director of activities. Her work truly goes unnoticed! She pours her heart out in every event for the residents. You can always catch her at the facility going up and down taking care of things. lol Alongside her team. All her staff is great. Miss Tess , Alyssa, Tiffany, Chelsea, ETC … if you are looking for a place that gets your loved one out of the bed and engages with activities this is the place to go. Communication is key and this place has delivered those needs. I would also like to recognize the staff – Ms Anne. Awesome worker – alongside Thomas who is trying his best to get my husband’s appt needs in order. The receptionist at the front who always pick up our phone calls so positively. My husband is on the subacute care side and all the interactions that we’ve had with the LVN RN CNA RT AND SUPERVISORS have been pleasant, like I’ve mentioned before – a couple misunderstandings once in a while but everything gets situated and taken care of. Again, thank you for all you’ve done and continue to do for my husband and his care.

Yessenia, where do I start. Thank you for everything you did for my father. I know at times he could be persistent and demands attention immediately but you never put him aside you put your work aside to see what Rudy needed. I’ve never had someone who wasn’t family to give him that attention. You always knew what to do to calm him down and make him feel happy till my mother arrived. Simple things like going to his room to get him for bingo, arts an crafts etc. putting money out of your pocket to make sure ALL patients enjoy their activity time (not many people do that) and holidays. Even had singing time because my dad loved to sing. I will forever be grateful to you and a lot of employees that work there. Keep being you because it doesn’t go unnoticed.

My brother is a client at country Oaks. Your activities department is the best. They are interacting with all the clients no matter what their disabilities are . Thinking out of the box always. 4 star. My Brother is Marvell.

My dad was at Country Oaks and his walking really improved. He had fallen and after receiving physical therapy he was able to return home and go up and down the stairs. They made sure he was well taken care of. The staff was accommodating. I recommend people looking for physical therapy to come here.

A case manager suggested I go for a tour of this facility yesterday. Though hesitant to send my loved one to any skilled nursing facility, I saw that Country Oaks Care Center did have a high Medicare star rating. Upon arriving I was pleasantly surprised by helpful, welcoming staff. Their admissions person gave me a tour and answered all the questions I had in addition to offering some additional help with questions I had regarding discharge from the hospital. I now understand that my loved one can benefit from the additional hours of physical therapy here. I also learned that I can visit whenever I like to ensure my family feels comfortable. Thankfully I also had a chance to speak to their head nurse who made me feel very comfortable. I am now feeling more confident we can get the help we need.

A long time friend and Dr. has been following his patients here & I recently had the opportunity to see why. This rehab is not only beautiful, it has the most compassionate staff! I’ve been very happy with the care given and the excellent customer service accommodations  especially with the restrictions of covid 19. I feel very assured that the best possible care is being provided. Thank you, Dr. Singh!

My mother has been a patient at Country Oaks for 2 1/2 months now. I visit her nearly every day, and I have observed the staff taking very good care of her and all of the other patients. Everything is clean, the staff is responsive to the needs of the patients, they haved organized social activities often, the meals and snacks are nutritious and appetizing. She is happy here. I recently went on vacation and I was able to get much needed rest and relaxation because I knew I had left her in good hands.
